Applications

Applications may be submitted throughout the school year. A $25 application fee is due with each application. Families currently enrolled, past patrons, and church members are given first priority during the month of September.  Open enrollment begins October 1.

Contract

A legally binding agreement between Trinity Preschool/PMO and the family of enrolled students. A $200 non-refundable deposit is due with each contract.

Handbook

A helpful resource of school policies and procedures.

A Parent Handbook Acknowledgment Form must be signed by the child’s parents each year.

VA School Entrance Health Form

The Commonwealth of Virginia School Entrance Health Form is required by Virginia State Law. It must be completed each year by both the child’s parents and physician prior to the child entering school.

Food Allergy and Anaphylaxis Emergency Care Plan

For a child with an allergy requiring epinephrine, this form needs to be completed by the child’s parents.

Medication Consent Form

One form must be completed for each medication. Multiple medications cannot be listed on one consent form. This form is not required for over-the-counter diaper cream, sunscreen, insect repellant, lotion, lip balm or Vaseline.
